The History of Health Care

Ok, so we, as a country, are about to spend billions of dollars on Short term health and Medical insurance plans for every citizen. Yet I wonder what is the history of health insurance? Why did it even come into existence? I imagine some smart accountant thought it up a long time ago, but I don't have a clue. When was the first health insurance policy written? What was and wasn't covered? I think a look back at the evolution of healthcare may help a lot in this debate.
